For this project, I developed a comprehensive clinical case study based on a patient encounter during my clinical rotation. The assignment focused on applying the Nutrition Care Process (NCP) to assess and manage a patient with Type 1 Diabetes and a diabetic foot infection.

I conducted a detailed nutrition assessment, analyzing the patient’s medical history, laboratory values, anthropometric data, and dietary intake. Using this information, I created an individualized nutrition care plan designed to support glycemic control, promote wound healing, and improve overall nutritional status.

In addition, I provided patient-centered nutrition education tailored to the patient’s condition, lifestyle, and readiness to change. This included guidance on carbohydrate management, balanced meal planning, and practical strategies to support long-term behavior change.

This project highlights my ability to integrate clinical data into evidence-based nutrition interventions, apply critical thinking in complex cases, and communicate effectively with patients to improve health outcomes.
